In this Privacy Policy (“Policy”), we describe how Oncology VIP Inc. ( “we,” “us,” or “our”) collects, uses, and discloses information that we obtain about visitors to our website www.oncadvisor.net (the “Site”), the services available through our Site (collectively, the “Services”), and information we obtain offline. By visiting the Site, or using any of our Services, you agree that your personal information will be handled as described in this Policy. Your use of our Site or Services, and any dispute over privacy, is subject to this Policy and our Terms of Use , including its applicable limitations on damages and the resolution of disputes. The Oncology VIP Inc. Terms of Use are incorporated by reference into this Policy.

 

The Information We Collect About You

We collect information about you directly from you and from third parties, and automatically through your use of our Site or Services.

Information We Collect Directly from You. The information we collect from you depends on how you use our Services. When you create an account on our Site, you are required to provide the following information based on your account type:

If you are an oncologist, you must provide your name, email address, address, phone number, National Provider Identifier (NPI), and may include states in which you are a licensed physician, board certifications, and practice areas;

If you are an individual receiving cancer treatment, you must provide your name, email address, address, treating oncologist, and may include details about your cancer diagnosis and treatment line; and If you are a family member of an individual receiving cancer treatment, you must provide your name, email address, address, phone number, name of the associated patient, your relationship to the patient, and may also include details about the patient’s cancer diagnosis and treatment line.

We also may collect additional information from you, including your mobile phone number, date of birth, gender, age, national or ethnic origin, your purchasing patterns or history, professional or business information, and any additional health information that you choose to share with us.

If you make a purchase, we will collect your payment information, such as your credit or debit card information and your billing address. We also collect information directly from you if you inquire about our Services through our Site or otherwise contact us.

Information We Collect from Other Sources. The information we may receive about you from other sources, including our affiliates, depends on how you use our Services.

If you are an oncologist, when we verify your professional or business credentials, we may receive list any personal information received from a third party (e.g., employer), including such information that may be publicly-available; and if you are an individual receiving cancer treatment, we receive your medical information, including your name, email address, address, treating oncologist, and if you authorize the release of this information, details about your cancer diagnosis and treatment line.

Information We Collect Automatically. We automatically collect information about your use of our Site through cookies, web beacons, and other technologies that operate on the Site. These technologies may collect your domain name, internet browser and operating system, the webpages you view on the Site, links you click on the Site, your device ID, the length of time you visit our Site and use our Services; your geolocation; and other interactions with our Site and Services. To the extent permitted by applicable law, we combine this information with other information we collect about you, including your personal information. Please see the section “Our Use of Cookies and Other Tracking Mechanisms” below for more information.

 

How We Use Your Information

For Service-Related Purposes:

  • To provide our Services to you;
  • To respond to your inquiries and communicate with your regarding our Services;
  • To process your payments and provide technical support;
  • To facilitate your ability to use the account portal; and
  • To offer personalized help and to otherwise personalize your experiences while using the Services.

For Marketing Purposes:

  • To send you promotional emails and text messages about our Services;
  • To send you news and newsletters; and
  • To offer our customer loyalty programs.

For Research, Product Improvement, Enhanced User Experiences:

  • To better understand how users access and use our Services, on an aggregated basis. For example, we will evaluate which features of our Site are more (or least) used by users, and we will use this information to make updates to our Site and Services;

How We Share Your Information

We may share your information, including personal information, as follows:

Service Providers. We may use service providers to help operate this Site and deliver our Services, and we may share your information with service providers that provide services for or through this Site on our behalf. We currently use service providers to assist with IT hosting and similar services, customer service, contact management information, document storage, payment processing and other operational functions.

Oncology VIP Inc. Users. Your user name and any information that you post to our Site, including, without limitation, comments and text will be available to, and searchable by, all users of the Site and Services.

Our Affiliates. We may share your personal information with our subsidiaries and/or affiliates.

We also disclose information in the following circumstances:

Business Transfers. If (i) we or our affiliates are or may be acquired by, merged with, or invested in by another company, or (ii) if any of our assets are or may be transferred to another company, whether as part of a bankruptcy or insolvency proceeding or otherwise, we may transfer the information we have collected from you to the other company. We disclose your information when we believe it is appropriate to do so to investigate, prevent, or take action regarding illegal activities, suspected fraud, situations involving potential threats to the safety of any person, violations of our Terms of Use or this Policy, or as evidence in litigation in which we are involved.

At Your Request. In addition to the above, we may disclose your personal information to third parties that you direct us to disclose your information to.

Other Information. Aggregate, anonymized, or de-identified information with third parties for marketing, advertising, research or similar purposes.
 

Our Use of Cookies and Other Tracking Mechanisms

We and our third party service providers use cookies and other tracking mechanisms to track information about your use of our Site or Services. We may combine this information with other personal information we collect from you (and our third party service providers may do so on our behalf).

Cookies. Cookies are alphanumeric identifiers that we transfer to your device’s hard drive through your web browser for record-keeping purposes. Clear GIFs, pixel tags and other technologies. Clear GIFs are tiny graphics with a unique identifier, similar in function to cookies. We may use clear GIFs (a.k.a. web beacons, web bugs or pixel tags), in connection with our Site and Services to, among other things, track the activities of Site visitors, help us manage content, and compile statistics about Site usage. Currently, our systems do not recognize browser “do-not-track” requests. You may, however, disable certain tracking as discussed in this section (e.g., by disabling cookies); you also may opt-out of targeted advertising by following the instructions in the Third Party Ad Network section.
 

User Generated Content

We invite you to post content on our Site and Services, including your comments, pictures, and any other information that you would like to be available on our Site or Services. If you post content to our Site or Services, all of the information that you post will be available to all registered users on our Site or Services. If you post your own content on our Site or Services, your posting may become public and Company cannot prevent such information from being used in a manner that may violate this Policy, the law, or your personal privacy.
 

Third-Party Links

Our Site and Services may contain links to third-party websites. Any access to and use of such linked websites is not governed by this Policy, but instead is governed by the privacy policies of those third party websites. We are not responsible for the information practices of such third party websites.
 

User Generated Content

We have implemented reasonable precautions to protect the information we collect from loss, misuse, and unauthorized access, disclosure, alteration, and destruction. Please be aware that despite our best efforts, no data security measures can guarantee security.

You should take steps to protect against unauthorized access to your password, phone, and computer by, among other things, signing off after using a shared computer, choosing a robust password that nobody else knows or can easily guess, and keeping your log-in and password private. We are not responsible for any lost, stolen, or compromised passwords or for any activity on your account via unauthorized password activity.
 

Access To Your Personal Information

We invite you to post content on our Site and Services, including your comments, pictures, and any other information that you would like to be available on our Site or Services. If you post content to our Site or Services, all of the information that you post will be available to all registered users on our Site or Services. If you post your own content on our Site or Services, your posting may become public and Company cannot prevent such information from being used in a manner that may violate this Policy, the law, or your personal privacy.
 

What Choices Do I Have Regarding Use of My Personal Information?

We may send periodic promotional emails or informational text messages to you. You may opt-out of promotional emails by following the opt-out instructions contained in the email. Please note that it may take up to 10 business days for us to process opt-out requests. Children Under 13
